35x45mm, 80% face, white background, matte finish, taken within last 6 months
☐ NID Card / Birth Certificate
Original + photocopy, translated to English/Polish by certified translator
☐ Marriage Certificate (if applicable)
Translated to English/Polish if in Bengali

☐ Work Permit (Zezwolenie na pracę)
Original issued by Polish Voivodeship, valid for the specific employer and position
☐ Employment Contract / Job Offer Letter
Signed by Polish employer, stating salary (≥ minimum wage), position, duration
☐ Cover Letter
Addressed to Polish Consul, explaining purpose, duration, accommodation, financial means
☐ Bank Statement (Last 3-6 months)
Stamped by bank, showing sufficient funds (≥ 500 PLN per month of stay)
☐ TIN Certificate & Tax Return
Last 1-2 years tax return acknowledgement from NBR Bangladesh
☐ Educational/Professional Certificates
Degrees, diplomas, training certificates relevant to the job, translated to English/Polish

☐ Accommodation Proof
Rental agreement, hotel booking, or employer-provided housing letter for entire stay
☐ Flight Reservation
Round-trip dummy/actual ticket matching visa application dates
☐ Travel Health Insurance
Minimum coverage €30,000, valid for Poland/Schengen, covering entire stay duration

☐ Police Clearance Certificate (PCC)
From Bangladesh Police HQ, issued within last 6 months, translated to English/Polish
☐ Medical Certificate
Fitness certificate from recognized hospital, stating no contagious diseases
